Frequently asked questions

FollowMyHealth is an app that allows patients to access and manage their personal health records online. This is known as the patient portal. Patients can also use the portal to track personal health goals and monitor progress of those goals.

With FollowMyHealth, you can:

  • Review hospital discharge instructions
  • View a variety of test results
  • View your medication and pharmacy info
  • Message your physician directly

FollowMyHealth provides access to your personal health records online as well as messaging communication with some of your providers.

With FollowMyHealth, you can view:

  • Inpatient/outpatient data
  • Upcoming appointments
  • Past appointments

FollowMyHealth features vary based on hospital location and physician practice. For specific information, please contact your provider and/or hospital directly.

FollowMyHealth and Northwell Health are two separate applications, each with its own unique features to help you manage your medical care. You can access both via web or your smart device.  See the table below to better understand the differences between the two applications.

Create or log in to FollowMyHealth

When you request to be an individual’s health care proxy, you are requesting to be given the power or authority to access their health care data.

If you become a health care proxy via FollowMyHealth for someone, you will be able to see their medical information, message providers and more within the portal.

  • You can be a FollowMyHealth proxy for:
    • Children from the age of birth through 12
    • Adults 18 and older

Children 13 through 17 are not eligible for proxy access.

Dependents must be between the ages of birth and 13, or 18 or older. (Children 13 to 17 are not eligible for proxy access).

To request proxy access for a dependent, complete the proxy request form below. When it is completed and signed, email form to [email protected]. It can take up to 48 days for approval after form is submitted. Once the form is approved, you will receive an email to the email address provided within the proxy request form, which will include a proxy invite link.

Proxy forms: (Note there are two separate proxy forms)

You can gain access to your child’s health information via FollowMyHealth by following the steps below:

  • Dependents must be between the ages of birth and 13 or above the age of 18. (Children within the ages of 13 to 18 are not eligible for proxy access)
  • To request proxy access for a dependent please complete the ‘Proxy request form’ below.
  • Once the form is completed and signed the form must be emailed to [email protected]
  • Once the form is approved, you will receive an email to the email address provided within the proxy request form, which will include a proxy invite link.

You can install the free FollowMyHealth app from the iTunes App Store or Google Play store. Once you have the app, you can choose to specify certain notifications be sent via text message to your mobile phone. Text messages will not contain protected health information.

You may request an amendment to your medical record if you believe that information in your record is inaccurate. Subject to your health care provider’s discretion and applicable law, we will do our best to accommodate all reasonable requests. To request an amendment, please complete and submit the Request for Amendment of Protected Health Information form.

If your request is regarding hospital records, check our medical records page to determine the correct location, email and phone number to submit your request.

Please note: Our medical records team is currently updating their contact information. If an amendment to your Physician Partners health record is needed, fill out the amendment form and email it to [email protected]. Once they receive the form, they will confirm receipt and send to the appropriate provider(s) for processing. A written resolution of your request can take up to 60 days from date of receipt to be received.

Some labs may not display within FollowMyHealth because of the facility at which they were drawn, or a problem with your FollowMyHealth account’s connection to a specific lab.

*Some providers’ locations may process their lab work through a lab that does not share their lab results with FollowMyHealth. View locations that do not share their results with FollowMyHealth. If the labs you are seeking were processed at a non-participating lab, contact your provider or the lab directly for your results.

*Lab results display within your account based on relationships that are created between your account and specific labs. This relationship may not yet be established between the lab results you are seeking and your account. You can contact (888) 321-DOCS, option 2, for additional assistance.

Your Northwell account

A Northwell account (sign up or log in) provides access to online features that will save you time and effort.

With your account you can easily:

  • View, schedule and manage appointments
  • Find pre-visit forms
  • Manage your account preferences
  • View and pay bills

Your account home connects you to FollowMyHealth, which works hand in hand with your Northwell account to give you more access to your medical records from Northwell and other medical providers.

Proxy access for minor patients

If you are the parent or legal guardian of a Northwell Health patient who is under the age of 13, you can request proxy access to your child’s health information in the FollowMyHealth patient portal. Proxy access enables you to view some of your child’s health information in the portal and, in some instances, communicate through the portal with your child’s health care team. When your child turns 13, you will no longer be able to view any new information about your child in the portal.

At this time, proxy access is not available for children between the ages of 13 and 18 years old.

Proxy access for adult patients

Through the FollowMyHealth patient portal, registered users can request online access to medical information for an adult in their care who has impaired decision-making abilities. Proxy access enables you to view the adult patient’s health information in the portal and, in some instances, communicate through the portal with the adult patient’s health care team.

To request proxy access for an adult, download and complete the form, and email it to [email protected].

FollowMyHealth mobile app

Enjoy the convenience of accessing your medical records and health information while away from your computer via your iPhone/iPad or Android device, including fax and email capability. If you currently use the FollowMyHealth patient portal to connect to our hospitals on your computer, this app extends that accessibility to your mobile devices. Use your same login to access your Universal Health Record account with your mobile device or tablet and take control of your health and wellness.
