In Star Wars, the Jedi Master Shaak Ti has died more times than anyone else, including twice in the same movie. For those who don't know or perhaps may not remember, Shaak Ti was one of the Jedi Masters who sat on the Jedi Council during the Clone Wars. She not only appeared in the Star Wars prequel movies (played by Orli Shoshan) but the Clone Wars animated series as well. Unfortunately, she was one of many who didn't survive the Jedi Purge (aka Order 66).

Just as most of her fellow Jedi Masters and Knights, Shaak Ti fought the Separatists and their droid army during the Clone Wars, including taking part in the Battle of Geonosis in Star Wars: Episode II - Attack of the Clones. After that, she went on to train clone troopers on the planet of Kamino, even going as far as to defend the training facility in the Battle of Kamino when it was attacked in the Clone Wars series. But despite all this, she was betrayed and killed... multiple times.

It seems that the first of many Shaak Ti deaths was supposed to happen towards the beginning of Star Wars: Episode III - Revenge of the Sith, when General Grievous had captured Shaak Ti and taken her aboard the Invisible Hand. Later, Grievous and his droids cornered Obi-Wan Kenobi and Anakin Skywalker, which is when Grievous executed Shaak Ti. But before she died, she said, "I'm sorry, Master Kenobi. I failed." Interestingly, this is one of two deaths that could've happened in Revenge of the Sith.

Genera Grievous kills Jedi Master Shaak Ti in frontof Anakin and Obi-Wan ina Revenge of th Sith Deleted scene

In another scene towards the end of the film, Shaak Ti is seen meditating in the Jedi Temple when Anakin (now Darth Vader) walks in and stabs her in the back, after she asks, "What is it, Skywalker?" (A version of this scene was later referenced in an episode of Clone Wars season 6.) While this fell in line with all the other Jedi being murdered after Palpatine initiated Order 66, this particular scene would've raised some questions regarding Shaak Ti's connection to the Force if it had made its way into the final cut. After all, she was meditating in the same building where younglings were being slaughtered in droves, yet she did nothing to help them.

Either of the scenes from Revenge of the Sith could've worked for Shaak Ti's canonical death, but the thing is, she died two more times in Star Wars Legends. Similar to her Jedi Temple death in the deleted scene from Episode III, Shaak Ti is killed at the same point in LEGO Star Wars: The Video Game, though this time Darth Vader cuts off her head rather than stab her in the back. Lastly, in Star Wars: The Force Unleashed, Shaak Ti attempts to use Felucia's plants to defeat Starkiller, but instead, Starkiller uses the Force to turn the plants against her. So, in total, Shaak Ti has died four times in the Star Wars franchise.
