Medical Guardian lands in one of the top three spots of our Best Medical Alert Systems rating and also lands in the middle of our rating for best systems with automatic fall detection.
While the company claims its home systems have a range of up to 1,400 feet, it was only found to work within a range of 400 feet during our testing. However, this was the best result out of all the systems we tested.
Additionally, in testing we failed to receive push notifications through the MyGuardian app when testing both the home and mobile systems, as they aren’t offered.

Medical Guardian’s medical alert systems are available for purchase either online or over the phone. Devices can be shopped individually through the company’s website, or customers may call and speak with a Life Safety Consultant for help choosing the right products and customizing a plan.
Depending on the equipment and subscription plan selected, there could be an upfront device fee of $149.95 or $199.95 plus shipping costs of up to $12.50.

We didn’t have any problems setting up Medical Guardian’s in-home or mobile medical alert equipment. We found the process to be very simple and straightforward, and it took less than 10 minutes for us to complete setup for each system. We also were able to connect the base unit with other devices on our first attempt.

Customers can also add to their system with wall buttons, extra help buttons, lockboxes, and Elite 911 speaker boxes. The company also offers add-on services like automatic fall detection monitoring, OnGuard Alerts, and Support Circle Apps. All systems come with the free MyGuardian app.
Medical Guardian is accredited by the Better Business Bureau and has an A+ rating, but it also has a low customer reviews BBB rating.
Medical Guardian’s U.S.-based monitoring centers are open 24/7 to take emergency and nonemergency calls from customers. These facilities have received multiple certifications that show they meet industry standards for training.
The company claims to connect customers to an operator within seconds. While this was the case in most instances during our testing of automatic fall detection response times, we did experience some inconsistencies. In all three tests of the home system, the alarm was triggered in less than five seconds. This was the same result in the first and third tests of the mobile system, but in the second test, the alarm never activated.
With in-home range testing, Medical Guardian’s MGHome Cellular system had the longest range of all the systems we tested, reaching 400 feet from the help button to the base station. However, the company’s advertised range is 1,400 feet.

Medical Guardian offers two home systems and three mobile systems. These are priced from $29.95 to $44.95 per month plus upfront device fees up to $199.95. With all five packages, customers will receive access to a user portal, wearable devices, and 24/7 UL-certified monitoring centers. Although the company’s upfront equipment costs make it one of the pricier options on the market, it has a broad selection of features and add-on services that help to provide a more complete medical alert package.
The setup fees for Medical Guardian systems differ based on the product and plan. The MGClassic device comes free with signup, while all other equipment must be paid for upfront. The company also offers monthly, quarterly, or yearly billing on subscription plans, with a 16 percent discount, free month, and free ground shipping when paying annually. Fees include:

Medical Guardian has several features that customers have the option to add to create a more comprehensive system:
Medical Guardian offers an automatic fall detection feature that can be worn around the neck via a pendant or as part of a device (MGMini), or around the wrist (MGMini Lite). It’s $10 per month and can be added to any of the home or mobile systems.
These devices are designed to sense when someone is in a falling motion and will automatically place a call to the monitoring center for help. It’s always a good idea to press the help button if you’re able to, though, since no automatic fall detection device is accurate all of the time.
The company claims its fall detection technology is highly accurate with a low risk of producing false alarms.

In our testing, the MGHome Cellular system did very well, registering a fall within five seconds in each of the three testing rounds. The MGMini system, however, worked well in two rounds, also triggering a fall alert within five seconds, but no fall was detected in the third round of testing.

Medical Guardian and LifeFone both offer in-home and mobile medical alert systems with similar subscription pricing. The main difference in cost is that LifeFone doesn’t charge any upfront device fees, whereas Medical Guardian does, and LifeFone provides fall detection for $5 a month compared to Medical Guardian’s $10 cost.
Another distinction is the availability of features like wellness checks, activity tracking, and medication reminders. These are included in each of LifeFone’s plans, while Medical Guardian doesn’t have wellness checks and only includes the other elements in some of its plans.
However, our testing showed where Medical Guardian really stood out – its signal range. Its MGHome Cellular system gave the best results among all the companies tested, reaching a range of 400 feet, while LifeFone’s At-Home Cellular system came in one of the last spots, performing consistently at only 80 feet.
In addition, with our three rounds of automatic fall detection testing, both companies faltered in one of the rounds. Medical Guardian’s at-home device performed well, but its mobile device wasn’t triggered in one of the rounds. Conversely, LifeFone’s At-Home & On-the-Go, VIPx device performed very well, triggering an alarm within five seconds, but its At-Home Cellular system took longer (up to 25 seconds) to trigger an alarm and also didn’t trigger a fall in one round of testing.
For those who are prone to falls, Medical Guardian may be the most reliable system, but LifeFone might be a better solution for customers who want access to the extra features.

ADT Medical Alert has just three product options to choose from compared to Medical Guardian’s five. The monthly subscription plans are in the same price range for both companies, but ADT Medical Alert charges slightly more per month for fall detection and it doesn’t offer it on the home landline-based system.
Aside from spouse monitoring, lockboxes, and wall buttons, ADT Medical Alert is lacking in the optional features it offers, while Medical Guardian has a varied selection.
Also notable is that in testing we discovered ADT Medical Alert doesn’t have a mobile app. Medical Guardian has the MyGuardian app, through which customers can manage many aspects of their system as well as connect with caregivers and emergency contacts.
ADT Medical Alert can be a good choice for those who prefer a basic system and don’t want the initial cost of equipment, while Medical Guardian is best-suited to tech-savvy individuals who like having device options and a customer portal that enhances the user experience.
In our testing, Medical Guardian’s MGHome Cellular system and ADT’s Medical Alert Plus and On-The-Go systems all triggered a fall in all rounds of testing within five seconds. However, Medical Guardian’s MGMini didn’t detect a fall in one testing round.
Range was much shorter for ADT’s Medical Alert Plus, reaching only 100 feet from the base unit, despite the system’s advertised range of 600 feet. While Medical Guardian’s MGHome Cellular system did test within range at 400 feet, its advertised range is 1,400 feet, which is a wider discrepancy.

Medical Guardian provides in-home and mobile medical alert systems with an assortment of devices designed to connect the customer to a monitoring center in the event of an emergency. This is done by following these steps:
Press the help button on your base unit, wristband, neck pendant, or wall button to reach a trained emergency response team within seconds.
Give a detailed explanation of your situation and let the operator know who they should reach out to for help, whether it’s a first responder, caregiver, or loved one.
After confirming an emergency, the appropriate personnel will be dispatched. If you’re unable to verbally identify your need, the monitoring center will automatically send medical services.
In nonemergency situations, the operator will notify someone from your designated care list.
U.S.-based monitoring centers are available around-the-clock to take calls from Medical Guardian customers. These facilities have received UL-Listed, Electronic Security Association, and Five Diamond certification. Live agents can also provide support in multiple languages. The monthly subscription fees for monitoring services range from $29.95 to $44.95.
Medical Guardian offers the MyGuardian app, which is included for free with each system purchase. It’s available to download on both iOS and Android devices, and it also can be viewed on desktop. Customers may use this to manage the people in their Care Circle, correspond with caregivers, track activities, review system reports, and take care of billing details. What features the app offers may depend on the system you purchase.
Medicare Part A and B plans don’t typically cover medical alert systems. However, Medicare Advantage (Part C) may provide coverage or reimbursement for your device. This would depend on your specific provider and plan.
Medicaid is run by states, so you would need to verify with your state’s Medicaid office if you’re eligible for any financial coverage for a medical alert system.
Medical Guardian issues prorated refunds if you’ve paid for services in advance but decide to cancel your subscription before that service time frame has ended. However, customers on a monthly billing plan won't receive any refund if canceling service within 90 days of sign-up. If sending equipment back, you may be charged a restocking fee of up to $50.
A limited one-year warranty applies to equipment purchased directly through Medical Guardian. This covers the replacement of products when issues arise due to defects in the workmanship or materials. The company may repair or replace equipment after that for the same type of defects, but not for normal wear and tear or damage.
A protection plan is also available for $6.99 a month to cover normal wear and tear, accidental damage, and lost or stolen devices, as well as unlimited battery replacements.
Customers have the ability to cancel their Medical Guardian subscription at any time. However, those on monthly billing plans won't receive any refund if they cancel within 90 days of starting service. Cancellations can only be done by calling and speaking with a company representative. Once the cancellation request is complete, equipment must be returned within 30 days to avoid a fee of $350.
