    SUMMARY

    Substance Abuse Comprehensive Outpatient Treatment (SACOT) Program means structured individual and group addiction activities and services that are provided at an outpatient program designed to assist adult consumers to begin recovery and learn skills for recovery maintenance.

    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned as necessary.

    • Individual counseling and support;
    • Group counseling and support;
    • Family counseling, training or support;
    • Biochemical assays to identify recent drug use (e.g. urine drug screens);
    • Strategies for relapse prevention to include community and social support systems in treatment;
    • Life skills;
    • Crisis contingency planning;
    • Disease Management; and
    • Treatment support activities that have been adapted or specifically designed for persons with physical disabilities, or persons with co-occurring disorders of mental illness and substance abuse/dependence or mental/developmental disability and substance abuse/dependence;
    • Monitors the provision of services;
    • The development, monitoring and revising of the recipient’s person centered plan;
    • First responder for recipients on a face-to-face basis and also telephonically at all times (24/7/365), with capacity for face-to face emergency response within 2 hours;
    • Documentation;
    • Have the knowledge, skills and abilities required by the population and aged to be served;
    • Supervision of qualified professional, associate professional and paraprofessional;
    • Attend Meeting;
    • Training and Staff Development.

    STAFFING REQUIREMETNS

    • Persons who meet the requirements specified for CCS, LCAS and CSAC under Article 5C may deliver the SACOT program.
    • The program must be under the clinical supervision of a LCAS or CCS who is on site a minimum of 90% of the hours the service is in operation.
    • Clinical services may also be provided by staff who meet the requirements for Qualified Professional or Associate Professional for Substance Abuse according to 10A NCAC 27G.0104 under the supervision of a LCAS or CCS.
    • The maximum face-to-face staff to client ratio is not more than 10 adult consumers to 1 QP
    • Paraprofessional level providers who meet the requirements for paraprofessional status according to 10A NCAC 27G .0104 and who have the knowledge, skills and abilities required by the population and age to be served may deliver SACOT services under the supervision of a LCAS, CADC, or CCS. Paraprofessional level staff may not provide services in lieu of on-site provision to recipients by a qualified CCS, LCAS, or CADC.
    • Recipients must have ready access to consultation services when warranted by the presence of symptoms indicating a co-occurring non-substance related Axis I or II disorder. These services shall be delivered by a psychiatrist who meets requirements as specified in 10A NCAC 27G .0104.

    SERVICE TYPE/SETTING

    • SACOT is a periodic service that is a time-limited, multi-faceted approach treatment service for adults who require structure and support to achieve and sustain recovery. Service emphasizes reduction in use and abuse of substances and/or continued abstinence, the negative consequences of substance abuse, development of social support network and necessary lifestyle changes; educational skills, vocational skills leading to work activity by reducing substance abuse as a barrier to employment, social and interpersonal skills, improved family functioning, the understanding of addictive disease, and the continued commitment to a recovery and maintenance program.
    • Services are provided during day and evening hours to enable individuals to maintain residence in their community, continue to work or go to school, and to be part of their family life.

    PROGRAM REQUIREMENTS/CLINICAL REQUIREMENTS

    • The SACOT program is offered at least 20 hours a week and offer a minimum of 4 hours of scheduled services per day, with availability at least 5 days per week with no more than 2 consecutive days without services available. The maximum face-to-face staff-to –client ratio is not more than 10 adults consumers to 1 QP based on an average daily attendance. The program must be under the clinical supervision of a CCAS or CCS who is on site a minimum of 90% of the hours the service is in operation.
    • The recipient must be in attendance for a minimum of 4 hours per day for any scheduled day of service. Group counseling services must be offered each day the program operates.
    • The development, monitoring, and revising of the recipient's person centered plan is the responsibility of the qualified professionals if the person was admitted to SACOT prior to the development of a PCP.
    • Must have policies and capacity to carry out first responder for their recipients on a face to face basis and also telephonically at all times (24/7/365), with capacity for face-to-face emergency response within 2 hours.
    • Staff shall demonstrate their knowledge and understanding of the service as well as demonstrate their clinical expertise and skill level by successfully completing a clinical interview.

    DOCUMENTATION REQUIREMENTS

    • Staff shall demonstrate their knowledge and understanding of a daily service note that includes: 1) the purpose of contact, 2) provider's interventions, 3) time spent performing the interventions 4) effectiveness of the intervention, and5) the signature (degree/credentials or position) of the person providing the service
    • Staff shall demonstrate their knowledge and understanding of completing a discharge plan which will be discussed with the recipient and filed in the medical record

    QUALIFICATIONS

    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.     EDUCATION and/or EXPERIENCE

    Qualified substance abuse professional (QSAP)" means, within the MH/DD/SAS system of care:

    (a) a graduate of a college or university with a masters degree in a human service field and has one year of full-time, post-graduate degree accumulated supervised experience in substance abuse prevention; or

    (b) a graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, post-bachelor’s degree accumulated supervised experience in substance abuse prevention; or

    (c) a graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, post bachelor's degree accumulated supervised experience in substance abuse prevention; or

    (d) a substance abuse prevention professional who is certified as a Certified Substance Abuse Prevention Consultant (CSAPC) by the North Carolina Substance Abuse Professional Practice Board.

    REASONING ABILITY

    Ability to solve practical problems and deal with a variety of concrete and abstract variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or diagram form.

    CERTIFICATES, LICENSES, REGISTRATIONS

    A CADC status shall be maintained for a period of up to five years while the registrant is in the process of completing his or her requirements for credentials. Must possess a North Carolina Substance Abuse Professional Practice Board Certification (Registrant CADC. Driver's license in good standing, car registration and vehicle insurance if providing transportation for individual’s receiving services. Possess current CPR/1stAide and NCI certification and BBP training, or complete training and certification in mandated timeframe.

    BACKGROUND CHECKS AND MEDICAL SCREENING

    Successful clearance of three (3) reference checks, statewide criminal background check and Healthcare Registry check. If a resident of NC less than five (5) years, successful National Background check completed by SBI and fingerprint

    clearance.
