Information about COVID-19 vaccines

Unity Health Toronto’s COVID-19 vaccination clinics are closed.

Eligible individuals can receive a COVID-19 vaccine at the following places:

  • Ontario’s vaccine booking system: https://covid-19.ontario.ca/book-vaccine/
    • Or call the Provincial Vaccine Booking Line number at 1-833-943-3900.
  • Visit https://ontario.ca/pharmacycovidvaccineto find a pharmacy offering a COVID-19 vaccine and book an appointment.
  • For those who are homebound – see information for accessing Toronto Paramedic Homebound vaccination
  • Vaccination text line – clinic search:
    • By texting VACCINE to 1-833-750-0888 and inputting your postal code, residents can receive a list of City-run clinics and a selection of the 450 pharmacies in the city offering vaccinations nearest to them.

Each clinic has its own eligibility criteria and appointment booking process. Please review each clinic’s page before making an appointment.

After your vaccination, you would have received a vaccine receipt by email. Be sure to check your junk mail folder as well.

If you are unable to locate your vaccine receipt, the Province has launched an online portal where individuals can access their vaccination receipt: https://covid19.ontariohealth.ca/

You will need the following information to get your receipt through this portal:

  • A green health card
  • Date of birth
  • Postal code

Missing or incomplete COVID-19 vaccination record?

If you have a green photo (OHIP) health card:

You can download your vaccine certificate from Ontario’s vaccination portal. To log in securely, you need:

  • green photo health (OHIP) card (you will need numbers from both the front and back of the card, expired cards will be accepted)
  • your date of birth
  • your postal code (the one associated with your health card)

If you do not have a computer or need help, call the Provincial Vaccine Contact Centre at 1-833-943-3900 (TTY for people who are deaf, hearing-impaired or speech-impaired: 1-866-797-0007) to request a copy of your vaccine certificate by email or mail.

If you have a red and white health card or do not have a health card:

Call the Provincial Vaccine Contact Centre at 1-833-943-3900 (TTY for people who are deaf, hearing-impaired or speech-impaired: 1-866-797-0007) and the contact centre agent can email you a secure URL to access your vaccine certificate.

Unity Health Toronto has been part of Ontario’s COVID-19 Vaccination Rollout since December 2020, with activities including:

  • Mobile outreach to congregate settings, shelters and First Nations, Métis and Inuit adults
  • Inpatient vaccination programs
  • Home visit initiatives
  • Running clinics at St. Joseph’s, St, Michael’s and Providence until July 30
  • Pop-up clinics supported by our West and Downtown East Ontario Health Teams

For the health and safety of our patients and our communities, even if you have received the COVID-19 vaccine, please continue to follow all public health measures and Unity Health Toronto’s visitor restrictions.  The COVID-19 vaccine protects against severe illness and death caused by COVID-19, but individuals who are vaccinated can still catch and spread the virus. Please continue to do the following:

  • Wear a mask, practice physical distancing and practice frequent and proper hand hygiene. For more information on our current public health measures, please visit this page.
  • Follow our current visitor policy which limits visits only to Essential Care Partners who have applied for, been accepted, and scheduled a visit as an Essential Care Partner.
