Amiloride is a WHO essential drug that has been registered in SA, but only in a combination product with too high a dose of
HCTZ. Use of amiloride in black and mixed ancestry patients with severe and resistant hypertension is highly likely to improve BP control, with resultant reduction in cardiovascular, stroke and kidney morbidity and mortality.
The literature search produced three relevant studies, all of which had large patients populations, published between 2008 and 2012, 'demonstrating an increased risk of SCC or lip cancer' associated with
HCTZ use, with over five years of use, the researchers wrote.
