Authorities released a crash report and video of a bus crash along I-70 in Licking County, Ohio that killed six people and injured over a dozen more earlier this month.

A semi-truck rear-ended a charter bus on Nov. 14 that was transporting members of the Tuscarawas Valley School District marching band to Columbus for the Ohio School Boards Association's annual convention.

The six who died in the crash were identified as:

  • John W. Mosley, 18, Mineral City
  • Jeffery D. Worrell, 18, Bolivar
  • Katelyn N. Owens, 15, Mineral City
  • Dave Kennat, 56, Navarre
  • Kristy Gaynor, 39, Zoar
  • Shannon Wigfield, 45, Bolivar

Troopers previously said another crash happened an hour before the tragedy.

At 7:51 a.m., the Ohio State Highway Patrol responded to the first non-injury crash. Reports indicate a semi-truck driver was following too close to the car ahead and collided with the vehicle. That triggered a chain reaction of two more cars. No one from this crash was severely injured.

The second crash involving the bus happened around 8:48 a.m. The crash report notes that the semi-truck struck an SUV, causing the SUV to strike the bus. The semi-truck overrode the SUV and struck the bus from the rear.

The crash caused the bus to collide with another SUV in front of it, which struck another semi-truck at the front and spun into the left lane. The bus then hit the semi at the front.

Troopers said the four vehicles at the front were slowing for traffic at the time of the crash.

A team of 16 from the National Transportation Safety Board, including nine investigators and two from its transportation disaster assistance team, are conducting a safety investigation.

The preliminary investigation report is expected within 30 days of the crash.
