Indications/Uses
Treatment of moderate to severe infections in adult & ped (≥3 mth of age) patients w/ complicated intra-abdominal infections due to E. coli, Clostridium clostridioforme, Eubacterium lentum, Peptostreptococcus spp, Bacteroides fragilis, Bacteroides distasonis, Bacteroides ovatus, Bacteroides thetaiotaomicron, or Bacteroides uniformis; complicated skin & skin structure infections, including diabetic foot infections w/o osteomyelitis due to Staph aureus (methicillin-susceptible isolates only), Strep agalactiae, Strep pyogenes, E. coli, Klebsiella pneumoniae, Proteus mirabilis, Bacteroides fragilis, Peptostreptococcus spp, Porphyromonas asaccharolytica, or Prevotella bivia; community acquired pneumonia due to Strep pneumoniae (penicillin-susceptible isolates only), including cases w/ concurrent bacteremia, H. influenzae (β-lactamase -ve isolates only), or Moraxella catarrhalis; complicated UTI including pyelonephritis due to E. coli, including cases w/ concurrent bacteremia, or Klebsiella pneumoniae; acute pelvic infections including postpartum endomyometritis, septic abortion & post-surgical gynecological infections due to Strep agalactiae, E. coli, Bacteroides fragilis, Porphyromonas asaccharolytica, Peptostreptococcus spp, or Prevotella bivia. Prophylaxis of surgical site infection following elective colorectal surgery in adults.
Dosage/Direction for Use
IV infusion for up to 14 days (infuse over a 30-min period) or IM inj for up to 7 days. Adult & ped patient ≥13 yr 1 g once daily. Ped patient 3 mth-12 yr 15 mg/kg bd, max 1 g/day. Duration of treatment: Complicated intra-abdominal infections 5-14 days. Complicated skin & skin structure infections, including diabetic foot infections w/o osteomyelitis 7-14 days. Community acquired pneumonia &/or complicated UTI, including pyelonephritis 10-14 days. Acute pelvic infections including postpartum endomyometritis, septic abortion & post-surgical gynecologic infections 3-10 days. Adult Prophylaxis of surgical site infection following elective colorectal surgery Single 1 g IV dose given 1 hr prior to surgical incision. Adult w/ severe renal impairment (CrCl ≤30 mL/min/1.73 m2) & ESRD (CrCl ≤10 mL/min/1.73 m2) 500 mg daily. Adult on hemodialysis Supplementary dose of 150 mg is recommended following hemodialysis session when recommended daily dose of 500 mg is given w/in 6 hr prior to hemodialysis.
Contraindications
Hypersensitivity to ertapenem, carbapenem antibiotics, & other β-lactams. Hypersensitivity to lidocaine or amide-type local anesth when administered as IM inj.
Special Precautions
History of hypersensitivity to penicillins, cephalosporins, other β-lactams & other allergens. Immediately discontinue if allergic reaction occurs. Caution w/ IM administration to avoid inadvertent inj into a blood vessel. Reports of seizures & other CNS adverse experiences, especially in patients w/ CNS disorders (eg, brain lesions or history of seizures) &/or compromised renal function. Reports of Clostridioides difficile-associated diarrhea. Prolonged use may result in overgrowth of non-susceptible organisms. Increased risk to develop drug-resistant bacteria if prescribed in the absence of a proven or strongly suspected bacterial infection or a prophylactic indication. Periodic assessment of organ system function, including renal, hepatic, & hematopoietic, is advisable during prolonged therapy. Not recommended w/ concomitant use of valproic acid/divalproex Na; probenecid. Patients w/ renal impairment (CrCl ≤30 mL/min). Pregnancy & lactation. Not recommended in infants <3 mth of age. Not recommended in treating meningitis in the ped population. No data in patients undergoing peritoneal dialysis or hemofiltration; ped patients w/ renal impairment &/or on hemodialysis.
Adverse Reactions
As treatment in adults: Diarrhea, infused vein complication, nausea, headache, vaginitis. As treatment in ped patients: Diarrhea, infusion site pain or erythema, vomiting. As prophylaxis of surgical site infection following elective colorectal surgery in adults: Anemia, small intestinal obstruction, pneumonia, post-op infection, UTI, wound infection, wound complication, atelectasis.
Drug Interactions
Increased plasma conc w/ probenecid. Decreased serum conc of valproic acid.
