I have to upload a negative PCR test result on the quarantine section of Visit Japan Web within 72hrs of my departure.

Can someone kindly clarify what exactly needs to be uploaded for the PCR section because I've had so many mixed responses.

Do I simply upload the original test certificate issued by the UK government approved test provider? (This certificate will satisfy the requirements as outlined on the Ministry of Health, Labour and Welfare Website).

Or do I upload a screenshot of a filled in version of page 1 of the following inspection certificate based on the results from my original test certificate: https://www.mhlw.go.jp/content/000799426.pdf

If I just upload a screenshot of a completed version of Page 1 of the inspection certificate, then how will they verify whether this is legitimate? Anyone can tick the required boxes and then upload the document?

I have been going back and forth with the Quarantine Documentation Center and have lost count of the number of email exchanges we've had. Their responses are vague. First they say to upload the inspection certificate and then they say to upload the original test certificate. Do I upload both or just one?
