Italy in OTL is unitary and the formation of a federation later implies a unitary status at game start, yet the name "Italian Union" implies a federal system.

I was curious about the term Union implying a federation.
So I looked at the wiki page for Union of the Comoros, and it says both,
“Government: Federal presidential republic”
And
“Politics of the Comoros takes place in a framework of a unitary presidential republic…”
It's been a long time since I played Italy, but if I remember correctly it's a unitary state at the start. And that's why the question of becoming a federal or confederal state shows up in the democratic paths.
