CONGRATULATIONS on your acceptance to South Miami Middle School's prestigious Magnet Program!



To fully process your acceptance to our school and to secure your placement into our Magnet Program, you need to:

1. LOG INTO THE DISTRICT SYSTEM TO ACCEPT YOUR SEAT ONLINE (yourchoicemiami.org)

2. SUBMIT YOUR DECLARATION OF ACCEPTANCE PACKET DIRECTLY TO SOUTH MIAMI MIDDLE SCHOOL.

PRINT, READ, SIGN and RETURN:

• The 3-page ACCEPTANCE LETTER below.

• The SUBJECT SELECTION form below that corresponds to the grade level that your child will be entering during the school year 2021-2022. (After printing, please have your child’s current teacher or counselor fill out the subject selection form before returning it to us.)




RETURN INSTRUCTIONS:

PLEASE RETURN ALL COMPLETED FORMS BY MARCH 31, 2021

• Emailed Packets accepted to jvaldesvega@dadeschools.net ONLY if sent from
the same email listed on the student’s application record. Please scan or take a
clear photo of each page of the packet in the order listed above with the Subject
Line reading “Acceptance – Program Auditioned For - Student ‘s Full Name –
Student’s ID #”
(Example: Acceptance - Visual Arts - Jon Doe - 1234567)

• In-Person Drop Off of packets accepted to SMMCS - MAGNET OFFICE - 6750
SW 60TH ST., MIAMI, FL 33143, Attention: Magnet Office.
