Staff was very unprofessional and spent most of the night bashing other staff. It was uncomfortable. They spent more time on smoke breaks third shift than they did with residents. A resident was screaming for help and they said he does that every night ignoring it. But I decided to check and the man was in distress at end of his bed. I was surprised to see this. It's a nice facility and has private rooms it's clean. But they need to really do something about the third shift nurses and aides included

Short of staffs !!! Not enough help. Because when my husband was there for a week and he kept asked for shower, NA said will do this later and never gave him shower. He say had shower 2 times before he was discharged. A few time, when he pushed call light and waiting for 10 to 15 minutes to answer his light. He wasn't to happy... Any way, I did research this place have 3 out of 5 stars... It was nice place and mostly private rooms.

2022... A nice place and clean. Large area in restroom and is perfect for someone in a wheelchair. The rooms are large enough for a bed, side table, and one chair. Each room has a window. The hall center seam of the carpet is raised up, but held down with carpet tape. Nice lighting and very quiet place. The personal have to let you in and out from 6 to 9. To leave, get a lady to unlock the door, as long hallways to walk. They have no push button call button for exiting, so have someone let you out. Close to Geneva, Ohio town.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
