A college might receive a number of forms on behalf of an individual applicant. These forms vary based on the recommendation requirements an college has elected and various actions by the applicant. Below, you'll find all possible forms that a college might receive.
School Report (SR)
Mandatory form for all Counselors/persons responsible for documenting a student's academic progress. Typically includes school profile and any transcripts.
School Report Transcripts (ST)
If desired, colleges can choose to separate transcripts from the School Report. This document will be referred to as ST, though still a part of the School Report submission.
Homeschool Supplement to the School Report (SR)
Students must invite their home school supervisor (ex. a parent) as their school counselors. Home school supervisors will then be asked additional questions within the standard School Report.
International Supplement to the School Report (SR)
International students should invite the individual responsible for their transcripts as their school counselor. Assigned school counselors will then be asked additional questions within the standard School Report.
Counselor Recommendation (CR)
Required or optional based on the college's requirements. Counselors may submit a recommendation even if it is not required by the college. The School Report must be submitted before the Counselor Recommendation.
Midyear Report (MR)
Includes midyear transcripts and should be sent when final grades for first semester or trimester are available. Required vs. optional status is selected by individual college.
Midyear Transcripts (MT)
If desired, colleges can choose to separate transcripts from the Midyear Report. This document will be referred to as MT, though still a part of the Midyear Report submission.
Optional Report (OR)
May be used at any point in the academic year to submit updated grades and/or transcripts. The OR can also be used to correct any errors previously submitted. It should not be used as a substitute for the Mid Year or Final Report and can only be submitted once an application year.
Optional Transcripts (OT)
If desired, colleges can choose to separate transcripts from the Optional Report. This document will be referred to as OT, though still a part of the Optional Report submission.
Final Report (FR)
The Final Report should only be submitted after a student has received all admission decisions. Any information that has changed since the submission of the School Report (SR) should be included, such as updated transcripts. Counselors have the option of submitting the report to the following college:
- The college at which the student has been accepted and intends to enroll.
- A college at which the student intends to enroll if admitted from the waitlist.
Final Transcripts (FT)
If desired, colleges can choose to separate transcripts from the Final Report. This document will be referred to as FT, though still a part of the Final Report submission.
Note: Once the Final Report has been submitted, the Counselor Recommendation, Midyear, and Optional Reports are no longer available to submit.
Teacher Evaluation (TE)
The Teacher Evaluation includes details about the teacher, their relationship with the student, and their letter of recommendation. If applicable, this will also include academic ratings for the student. Teachers are assigned to one of the following subject areas: Computer Science, English, Math, Science, Social Studies, World Language, Other. Teachers will only submit one, general, letter of recommendation per-student regardless of how many institutions the student is applying to.
Each college determines the number of required and allowed teacher evaluations.
Other Evaluation (OE)
Other Evaluations are provided by the other recommenders referred to in the application. Other evaluations are from non-academic recommenders and are assigned to one of the following areas: Arts Teacher, Clergy, Coach, College Access Counselor, Employer, Family Member, Peer, other. These recommenders will only submit one, general, letter of recommendation per-student regardless of how many colleges the student is applying to.
Each college determines the number of required and allowed other evaluations. Members can also determine the type of Other Recommenders a student is permitted to use to meet these requirements.
Fee Waiver (FW)
The Fee Waiver form is submitted by the Counselor in the event that a student has indicated that they qualify for the Common App Fee Waiver. In a student's application, they indicate their intent to use a CA Fee Waiver. This action triggers the FW form to appear in the counselor portal for this applicant. The form allows the Counselor to verify that the student qualifies.
Note: the FW is not required to waive the fee. The student's indication that they qualify is the only technical requirement for their fee to be waived.
Early Decision Agreement (ED) & (EDII)
For institutions that offer early decision admission plans, a student is required to agree to the binding agreement of early decision. Early decision intent is initiated by the student within the college's general section. The student is then required to sign the agreement themselves and invite a parent/legal guardian to sign. The Counselor will also see a form appear in the counselor portal for this applicant. The ED form received by the college includes both the counselor and parent/legal guardian signature.
