- During the orgy scene, there are a few shots of oral sex both gay and straight, and a few lesbian sex scenes.

- A man is suffocated with a cloth and he dies.
- Many people are whipped throughout.

- A man is stabbed many times and he staggers around then he is stabbed several more times by spears. Very bloody.

- The main character acquires his throne through manipulation and the intentional suffocation of the dying emperor. He slowly loses his sanity over the course of the film, and brings about many strange policies and decisions. Several characters' lives are in jeopardy at one time or another.
- There are several versions of this movie. Some versions are done through alternate angles and some cut violent scenes.
- R-rated version still has a lot of bloody scenes.
- Some may find Caligula's death upsetting.

- Caligula has an incestuous relationship with his sister.
- A man rapes a virgin bride in front of her husband. Because of the power dynamics, the husband does not resist.

- In the final scene of the movie, a small child gets murdered. Then, a man grabs her and stomps her head on the stone stairs of the palace.
