Does anyone know what the Attestation forms ask you for exactly.
Do you just give them your flight number and then upload your negative tests and that's it?
What about vaccination status??

Yes. Adults are to be vaccinated unless an exception.
And that unvaccinated children will be self tested days 3-5.

I went back to check. You attest regarding vaccination status but any vax proof is not inserted. You deal with that separately.
