Indications/Uses
Addisonian pernicious anaemia. Prophylaxis & treatment of other macrocytic anaemias associated w/ vit B12 deficiency. Schilling test.
Dosage/Direction for Use
Addisonian pernicious anaemia & other macrocytic anaemias w/o neurological involvement Initially: 250-1,000 mcg IM on alternate days for 1-2 wk, then 250 mcg wkly until the blood count is normal. Maintenance: 1,000 mcg mthly. Addisonian pernicious anaemia & other macrocytic anaemias, anaemias w/ neurological complications Initially: 1,000 mcg IM on alternate days. Prophylaxis of macrocytic anaemia associated w/ vit B12 deficiency resulting from gastroectomy, some malabsorption syndromes & strict vegetarism 250-1,000 mcg mthly. Schilling test 1,000 mcg IM.
Overdosage

Contraindications
Hypersensitivity. Not indicated for treatment of toxic amblyopias.
Special Precautions
Regular exam of blood is advisable. Folate metabolism should be investigated if megaloblastic anaemia fails to respond to therapy. Haematological response in patients w/ folate deficiency. Cardiac arrhythmias secondary to hypokalaemia during initial therapy. Monitor K plasma during this period. Not for treatment of megaloblastic anaemia of pregnancy.
Use In Pregnancy & Lactation
Adverse Reactions
Rarely itching exanthema, chills, fever, hot flushes, nausea & dizziness; anaphylactic shock.
Drug Interactions
Poor response in chloramphenicol-treated patients. Decreased serum conc w/ OCs. Invalidated vit B12 assay by microbiological techniques w/ antimetabolites & antibiotics.
