To apply for child care licensing, complete and return the application and forms for your program type: 

• Child Care Center, Preschool, and Head Start
• Licensed and Group Day Care Homes
• School Age Program
• School Age Drop-In Program
• Day Care Resource and Referral Agency
• Background and Registry Checks for Child Care Facilities (PDF) using form CCL 002a to determine the appropriate role for each affiliate.
  
  
• License fee currently waived through September 30, 2024. Local jurisdictions may charge fees unrelated to the Kansas Department of Health and Environment (KDHE).
