When creating the character, the name went through various combinations built upon the phrase “Dark Water”, with Lucas toying with different name combinations with last names like Wilson and Smith, until he landed on the winning combo of “Darth Vader”. In A New Hope, Obi-Wan Kenobi calls him “Darth”, suggesting it was his first name, as the whole Anakin Skywalker/Luke’s father concept wasn’t a thing yet, so “Darth” most likely began as a name. As the story progressed and the Star Wars universe expanded, “Darth” took a different meaning.

“Darth” is now a title given to the Sith Lords of the Sith Order, such as Darth Sidious (Palpatine), Darth Tyranus (Count Dooku), and Darth Maul. It was widely believed for years that “Darth Vader" meant “Dark Father”, even fueled by Lucas himself after the release of Star Wars: The Empire Strikes Back, but that’s not true at all as the “father” twist in that film wasn’t even planned when Darth Vader was created. Lucas has said “Darth” is a variation of “dark”, though he has also explained the inspiration behind the name were the words “death” and “invader”. There’s no actual meaning to the word “darth” in the real world, only the in-universe explanation of a Sith title. Like many other things in Star Wars, “Darth” has taken different meanings, and ended up becoming a title related to the dark side of the Force.
