Question
I completed the relics of the old faith DLC and the base game, however, I’m still missing some forms those being
Butterfly, mosquito, goldfish, rat, gorilla, moose, and then another one from darkwood (I can’t tell what it is behind the lock)
If anyone can help me get these, it would be greatly appreciated.

The Mosquito, Gorilla, Moose , Goldfish and possum are unlocked if you buy the heretic pack
For the butterfly form you need to catch the fireflies at night in your cult (the red glowing orb thingies)
For the one your not sure what's behind the locked follower, it is indeed a squirrel and in order to get this form you need to kill squirrels upon crusading in darkwood.
For the rest I'm not sure but hope that helped.
Any idea how many fireflies you need to catch before you're rewarded?
No idea, I don't think there's a specific number to drop so you need to do that as many till it does drop the follower form.

The butterfly you get by catching those red thingy’s that fly around the cult.
The one from darkwood can probably be the squirrel, you get it by killing squirrels in darkwood.
The others are from the paid DLC: the heretic pack.

Thanks for the help. for the butterfly are you talking about catching the butterflies with red wings? If so, how do you do it? I’m walking up to them, but there is no option to interact. Also, does this mean I am required to purchase the heretic pack for the platinum trophy on PlayStation?
You need to approach the glowing red fireflies and catch them by pressing x, it will not guarantee to get it from the first attempt so you need to catch many of them. For the heretic pack it is completely separate from the platinum trophy, you can get the platinum without buying any packs.
I meant the red dots at night, y’know, the red flying things?
You don’t need the DLC skins for the trophy, I think.
Also, you can only catch them inside of your cult, not in the area selection area.
