Any Filipino resident of Quezon City who is sixty (60) years of age or older can obtain an OSCA Card. This may apply to seniors with “dual citizenship status” if he/she can prove his / her Filipino Citizen status and has resided in this city for at least six (6) months.

Requirements:

1. Fully Accomplished QCitizen ID Online Form (https://qceservices.quezoncity.gov.ph/) or Request for Sectoral Status Change (if already have a regular resident/non-senior QCitizen ID)
   1. A printable form is also available online (https://quezoncity.gov.ph…pdf) for representative and onsite assisted encoding at the Office for Senior Citizens’ Affairs (OSCA)
2. 1 original and 1 photocopy of valid ID with applicant’s birth date and address (if no QCitizen ID)

3. 1 original and 1 photocopy of the Birth Certificate and Barangay Certificate of Residency (if there is no QCitizen ID or the birthdate and address are different from the Valid ID)

4. 1 original and 1 photocopy of the Marriage Certificate (if the applicant is a married woman whose last name is different from the Valid ID)

5. Latest digital or ID photo (2×2) – Colored with white background

6. A signature or can send a digital signature (clear and uncut on any part of the signature inside the box on the form)

Additional requirements for representatives:

 1.    Authorization Letter signed by the applicant

 2.   1 original and 1 photocopy of the valid ID of the representative (preferably QCitizen ID)

Process for Online Application:

1. Create a profile and account with QC E-Services, at this link – (https://qceservices.quezoncity.gov.ph/)
2. From the QC E-Services menu, select QCitizen ID eApplication
3. Fill in all required information or data. If you already have a resident/non-senior QCitizen ID, select Request for Sectoral Status Change
4. Wait for the email confirmation once your application has been approved
5. Download the QCitizen App on the smartphone to get a digital copy of your QCitizen ID
6. Wait for the physical ID to be delivered to your registered address

Process for Offline/Walk-in Application:

1. Go to the quezoncity.gov.ph website and download S.C. QCitizen ID Application Form or obtain a form from OSCA
2. Submit the Fully Accomplished Senior Citizen QCitizen ID Application Form and all required documents for OSCA verification
3. Once approved, the ID will be sent to the senior citizen’s home or can be pick-up at OSCA

The OSCA replaces a Senior Citizen’s Card that needs correction, updating, or is damaged, lost, or transferred from another city/municipality.

 Requirements:

 1.   Fully Accomplished Senior Citizen QCitizen ID Application Form

 2.  1 original and 1 photocopy of valid ID/documents showing the correct data or   Barangay Certificate of Residency (if no or different Quezon City address, starting at least 6 months residency) 

3.  Certificate of Cancellation from origin OSCA of city/municipality and surrender the old Senior ID (for the transferee to Quezon City)

Additional requirements for representatives:

1.   Authorization Letter signed by the Senior Citizen 

2.  1 original and 1 photocopy of the valid ID of the representative (preferably QCitizen ID)

Process:

1. Submit Fully Accomplished Senior Citizen QCitizen ID Application Form and all required documents for OSCA verification

2. Proceed to Cashier for the payment of Senior Citizen’s ID replacement (Php100.00)

3. Once printed, the ID will be sent to the Senior Citizen’s home or can be picked up at OSCA

This award is a recognition of legitimate city residents aged 100 years and older. Apart from the certificate, they will also receive benefits of Php100,000.00 (one-time), a monthly allowance of Php1,000.00, an annual birthday gift of Php1,000.00, and a Christmas gift of Php1,000.00 – as long as the Centenarian lives.

      Requirements: (can be applied for by representative)

1. Fully Accomplished Centenarian Application Form (can be downloaded online at https://quezoncity.gov.ph…pdf or obtain a form from OSCA)

2. 1 original and 1 photocopy of an Old OSCA Card or QCitizen ID issued at least 6 months

3. 1 original and 1 photocopy of Certified True Copy of Birth Certificate (PSA / CCRD) or Baptismal Certificate  

4. 1 original and 1 photocopy of two (2) of the following (if there is no Birth or Baptismal Certificate): 

                       a.  Passport (old or new)

                       b.  Postal ID

                       c.  Voter’s Registration or ID

                       d.  AFPSLAI / Veteran’s ID

                       e.  GSIS / SSS ID

                       f.  Philhealth MDR

5.   1 original and 1 photocopy of the Barangay Certificate of Residency (if the address is different from the Senior ID) 

6.   One (1) whole-body picture with the current newspaper

Process: (application can be made three months in advance, but verification will only begin when he/she has reached his 100th birthday)

1. Log in to quezoncity.gov.ph website and download/print Centenarian Application Form or obtain a form from OSCA

2. Submit the Fully Accomplished Centenarian Application Form and required documents to OSCA

3. OSCA staff will conduct a house visitation when the Centenarian reaches 100 years of age

4. OSCA will process the application and validate the documents to verify the information contained in the application

1. The applicant will receive notice about two or three (2-3) months after the verification when benefits will be provided for the Q.C. Centenarian Awardee

This is a program of the Department of Social Welfare and Development (DSWD) for indigent Senior Citizens who are frail, sickly, or with a disability and without a pension or permanent source of income, compensation, or financial assistance from relatives to meet their food and medicine needs. The QC Government and OSCA coordinate and only assist DSWD in the application process and payout of this program.

Requirements:

1. Original Senior Citizen QCitizen ID or OSCA Card

2. Fully accomplished DSWD Senior Citizen’s Social Pension Application Form is available at OSCA or can be downloaded (https://quezoncity.gov.ph…pdf)

Process:

1. Log in to quezoncity.gov.ph website and download/print DSWD S.C. Social Pension Application Form or obtain a form from OSCA

2. Go to OSCA and submit the Fully Accomplished DSWD S.C. Social Pension Application Form and present original QCitizen Senior ID or OSCA Card

3. DSWD staff will conduct a validation or home visitation at the applicant’s residence

4. If eligible, the name of the indigent Senior Citizen will be included in the list to be submitted by OSCA to DSWD

5. If approved by DSWD for Social Pension, OSCA will inform the applicant that he/she is included in the approved payroll list

6. Wait for notification when the Social Pension Pay-out will be made for approved indigent Senior Citizens beneficiaries

Reminder:

• OSCA only accepts applications and DSWD is the final assessment/approval if an indigent Senior Citizen is qualified.

• Not all eligible indigent Senior Citizens will be immediately qualified for DSWD Social Pension as it is only based on currently available funds for the program and priority list depending on age group or health status.
