Creo que en la mayoría de contextos se podrían usar indistintamente pero:
-¿A quién hablas? → Estamos varios y no sabemos a quién te diriges exactamente.
-¿A quién le hablas? → Te escucho hablar pero no veo a nadie más, así que te pregunto que con quién hablas.
