Certegy Payment Solutions, LLC ("Certegy"), formerly known as Certegy Check Services, Inc., provides check authorization and guarantee services to merchants and businesses nationwide. Certegy is a "nationwide specialty consumer reporting agency" governed by the Fair Credit Reporting Act (FCRA). You have the right under the FCRA to know the information contained in your file at Certegy. You also have the right to a free copy of your check transactions file from Certegy if you request it within 60 days of the date your check was declined. In addition, if you find that any information contained in the file from Certegy is inaccurate or incomplete, you have the right to dispute it with Certegy.

Identity Theft

If you have been a victim of identity theft or have lost/stolen checks, we recommend the following:

1. Visit www.consumerfinance.gov/learnmore or www.identitytheft.gov for helpful information regarding identity theft.

2. Review the Consumer Financial Protection Bureau’s Identity Theft Summary of Rights.

3. Contact all financial institutions where your accounts have been compromised.

4. File a police report and get a copy of the report to be used, if needed, to show proof of the identity theft.

5. To report identity theft to Certegy or initiate a fraud dispute for information contained in Certegy’s files, please send the following information to our Correspondence Department:

    • An original notarized Affidavit with your signature including notary stamp on the same page

    • A legible copy of your driver’s license

    • A copy of the police report (if available)

6. Order your Consumer File Disclosure Report, free of charge, which will allow you to determine if your personal information has been used at any of Certegy’s participating merchant locations. If you find any inaccuracies, contact us as soon as possible to file a dispute.
